This report has been prepared as part of the Australian Centre of Excellence for Local Government (ACELG) Rural-remote and Indigenous local government program. In 2011, ACELG released a report which found that the current demands and expectations placed on rural-remote and Indigenous local government councils could not be maintained, especially if they continued to grow unrestrained. The current report focuses on:

  • providing a profile of rural-remote and Indigenous councils from Western Australia, Northern Territory and Queensland, and identifying factors that impact on local capacity
  • a review of background information from past research and inquiries to identify federal, state and community expectations and issues from discussions with these stakeholders
  • the identification of further actions and research which could contribute to the objectives of ACELG.

Sad news, sorry business: guidelines for caring for Aboriginal and Torres Strait Islander people through death and dying.

This document provides general guidance for carers of Indigenous Australian people faced with death.
The guidelines take into account that for many Indigenous people, the times before death, of death and following death, are subject to a number of customary practices.

The first part of these guidelines covers cultural competence for health workers, including verbal and body language, building rapport, and the roles of Indigenous hospital liaison officers and Indigenous health workers.

The second part of these guidelines covers caring for Indigenous patients and their families during the patient's final stages of life. Topics are:

  • diagnosis and information
  • the gathering: preparation for death
  • time of death
  • sad news, sorry business: time after death
  • postponing the burial
  • financial barriers
  • traumatic or sudden death
  • open disclosure
  • coronial investigations
  • resource list.

The guidelines are intended for Queensland Health staff, but may be a useful resource for workers in other parts of Australia. The guidelines acknowledge that customary practices about death vary for different groups of Indigenous people, and that any document can provide general guidance only.

"Black engagement" Practical tips on how to engage with the Mt Isa Indigenous community (2011)

This two-page colour pamphlet provides rural placement students and health practitioners with practical tips about how to engage respectfully with Indigenous patients and colleagues in Mount Isa, Queensland. The tips in the pamphlet include:

  • do understand cultural time frames (Murri time) and be prepared to be flexible
  • do wear appropriate clothing
  • don't use inappropriate language (e.g. too much jargon or 'big words')
  • don't speak down or be condescending
  • don't make promises you can't keep.

The pamphlet was produced by the School of Medicine and Dentistry at James Cook University (JCU), in collaboration with the Mount Isa Aboriginal and Torres Strait Islander Reference Group (MIATSIRG). The aim of the MIATSIRG is to support JCU in delivering their services to Indigenous communities in a culturally appropriate manner, and to improve how JCU engages with Indigenous communities. Some of the general information in the pamphlet may be useful for health practitioners working in Indigenous settings outside of Mount Isa.

This report presents research undertaken by Queensland Health on the blood lead levels of children between one and four years of age living in Mount Isa, Queensland. It is a follow up study to one completed in 2007. The aim of the current study was to continue the monitoring of blood lead levels in this age group. The results revealed a significant decline between study surveys, however for Indigenous children, on average, they have higher blood lead levels. The report discusses the implications of the results.

This report describes the findings of a study into the incidence, nature, and outcome of contact with the juvenile justive system, for a cohort of 8,236 young people aged 10 to 16 years in Queensland between 2000 and 2007. The study was designed to assess whether greater use of diversionary measures, as opposed to formal sanctions, would reduce over-representation of Indigenous juveniles within the juvenile justice system. The study comprised three primary research topics: the amount/extent of contact with the juvenile justice system; processes used; and the impact of police diversion on further contact with the juvenile justice system. The study's findings indicate that Indigenous juveniles come into contact with police earlier, at a more frequent rate and are less likely to be diverted to informal measures than non-Indigenous juveniles.

This report describes the implementation and findings of a community based suicide prevention initiative in several Queensland Indigenous communities. The project used the knowledge and experience of the Indigenous community in Yarrabah as a model for other communities for the establishment of effective and sustainable community based approaches to build resilience, reduce suicide risk exposure and reduce self-harm. Through knowledge sharing, the skills, knowledge and experience of each community was strengthened.

The report documents the implementation of this project and discusses the various challenges encountered. This may have valuable implications for future Indigenous community based suicide prevention programs using an empowerment approach.

This report aims to inform policy, program development and practice regarding key directions for the development of a social, emotional, cultural and spiritual wellbeing framework relevant to Aboriginal and Torres Strait Islander Australians in Queensland. The report provides a rationale for focusing on social, emotional, cultural and spiritual wellbeing (SESWB) as a way to promote Indigenous health and wellbeing and reduce Indigenous disadvantage.

Three key directions for future investment are described: build evidence, enhance capacity and develop workforce. The report sets out a rationale, examples of current initiatives, and recommended actions for each key direction.

    • new approaches to service delivery
    • practitioner tools and mental health care
  • Part 2 provides guidelines for supporting people with the following mental illnesses:
    • depression
    • anxiety disorders
    • self-harming behaviours
    • schizophrenia and bipolar disorders.

This online resource for Environmental Health Workers is designed to support EHW employed by the local councils of Indigenous communities in Queensland.

It provides information on six key topics:

  • how to ensure the local community has safe drinking water

To help EHW find information quickly, the six key topics are broken into sections, such as:

  • the role of the EHW

  • agencies that can be contacted for more information or support

This publication outlines the plan for a strategic framework to improve the environmental health outcomes of Aboriginal and Torres Strait Islander people in Queensland. The plan builds on the progress made by previous strategies and recognises six areas to enhance environmental health outcomes for communities, including, to:

  • develop, implement and build on successful environmental health programs
  • engage with government and non-government community health and affiliated services
  • promote, develop and support the environmental health workforce
  • reinforce joint ventures across government and non-government organisations to support investment in environmental health services
  • promote environmental health programs and their success
  • develop and employ dependable reporting procedures to assist in the monitoring and evaluation of programs

The plan supports the objectives for the National Strategic Framework for Aboriginal and Torres Strait Islander Health.

The Rio Tinto Child Health Partnership was developed to deliver improvements in Aboriginal and Torres Strait Islander child and maternal health through the delivery of three projects:

* modelling the WA Aboriginal Child Health Survey for the Northern Territory and Queensland
* national foetal alcohol syndrome prevention strategy
* enhancing Aboriginal workforce capacity

The five-year Rio Tinto Child Health Partnership concluded in 2007. The report Delivering improvements in Aboriginal and Torres Strait Islander child and maternal health evaluates the results from the partnership, and states that the partnership has allowed strong relationships to be forged between researchers, Aboriginal communities, health services, government agencies and departments and corporate Australia. Significant benefits and outcomes include: increased awareness of fetal alcohol syndrome; identification of the issues which inhibit Aboriginal health professionals; support for culturally appropriate practices in primary health care and treatment services; and increased skills of health and community workers in identifying and responding to risk factors during pregnancy.

Compared with non-Indigenous men, Aboriginal and Torres Strait Islander men suffer from substantially more serious illnesses and early death due to problems arising from diabetes, heart disease, hypertension, cancers, respiratory diseases, psychological disorders, accidental injuries, violence and other causes. Despite this knowledge little is known to date about reproductive health among Indigenous men, with recent research being limited mainly to studies of sexually transmitted infections.

The aim of this research was to improve the understanding of sexual and reproductive health problems experienced by Indigenous men. A mixed method design combined qualitative inquiry (4 focus groups and 18 in-depth interviews) and quantitative survey (n=301) involving men living in remote, rural and urban communities (Tiwi Islands, Darwin and north and south-east Queensland). Survey data were compared with recently published self-reports from 5990 randomly selected men aged over 40 years in Australia (Holden et al., 2005, The Lancet, 366, 218-224). Findings revealed that most Indigenous men were silent about reproductive health but reported symptoms of erectile dysfunction at least as much as non-Indigenous men in other studies and were also found to have more symptoms of prostate disease than non-Indigenous men. Men's reaction to sexual difficulties included shame, denial, substance abuse and occasionally violence. The research provides insight and depth into the issues impacting on Aboriginal and Torres Strait Islander males experiencing reproductive and sexual health difficulties. The study also highlights issues concerning the layers of silence around sexual and reproductive health of Indigenous men, including silence in the scientific establishments in health services, and in the community. Implications for education of primary healthcare workers and community-based awareness campaigns for Indigenous males are discussed.

This report discusses the Cape York Welfare Reform Project which aims to develop reformed incentives and appropriate supports at the community level to help with the restoration of social norms. The core principles of the Project are:

* All welfare should be conditional
* Further government investment in capability building is needed
* Incentives need to be fundamentally changed to encourage people

Once the policies recommended by the Institute are supported, the Project will initially be trialled in four communities on the Cape York Peninsula. If the trial is successful, the policy principles may be used to inform policy across the Cape York Peninsula and potentially other regions.
