Description

Living every day with my heart failure is a resource for Indigenous people with chronic heart failure (CHF). It features culturally and clinically appropriate health information to help Indigenous people with CHF better manage their condition.

The booklet includes an action plan and medicines record sheet. Resources available in the package include:

  • my medicines
  • my fluid plan
  • my health record
  • my clinic and doctor visits.
