Health navigator

 

Overview

Health navigator is a Silver Chain program that uses telephone and telehealth technology to support clients to effectively self-manage their chronic conditions. Health navigator works in partnership with the client, their general practitioner (GP) and other health care providers, to provide an integrated and coordinated primary health service to the community.

Health navigator is available for people with one or more of three priority conditions for clients residing in the Wheatbelt and Central Great Southern regions of Western Australia:

Silver Chain provides a single point of access to Health navigator and links clients to the health care they need, when and where they need it. They also support clients to develop personalised plans with specific actions aimed at helping them better manage their chronic conditions.
