The Indigenous chronic disease package is the Commonwealth of Australia's $805.5 million contribution to the National Partnership Agreement on Closing the Gap in Indigenous Health Outcomes. The package aims to reduce key risk factors for chronic disease in the Indigenous community, improve chronic disease management and follow up, and increase the capacity of the primary care workforce to deliver effective care to Indigenous Australians with chronic diseases. This funding is provided in order to compensate for the shortfall in Medicare Benefits Schedule (MBS) expenditure for Aboriginal and Torres Strait Islander people, who experience a greater burden of disease.
