Improving the patient journey for Aboriginal and Torres Strait Islander peoples with Acute Coronary Syndrome

 

Overview

Join the Heart Foundation and Australian Healthcare and Hospitals Association (AHHA) to review best practice models for Aboriginal and Torres Strait Islander peoples experiencing Acute Coronary Syndrome.

The Heart Foundation and AHHA have been working together for over five years to reduce hospital disparities for Aboriginal and Torres Strait Islander peoples, particularly in the area of Chronic Heart Disease.

There is clear evidence that Aboriginal and Torres Strait Islander peoples experiencing Acute Coronary Syndrome do not receive the same level of care or achieve the same clinical outcomes as other Australians. It is equally clear that there are services that are addressing this issue at the local level. With support and collaboration the successes of these services can be expanded to improve health outcomes for the whole population.

This workshop will showcase examples of best practice and support the development of a structured collaboration program to translate effective programs to other hospital and health services.

The workshop program allows for interactive panel sessions as well as smaller workshop groups to examine key issues and develop an advocacy and project action plan.

This workshop is a delegate add-on to the Heart Foundation conference (Prevention of cardiovascular disease: translating evidence into practice). You are encouraged to stay on in beautiful Adelaide to experience the latest in heart health and heart care at the Heart Foundation's annual conference including an exciting program on Aboriginal and Torres Strait Islander cardiovascular health.

Free for Heart Foundation conference delegates, $30 non conference delegates.
