Service and support requirements of people with younger onset dementia and their families: final report, August, 2012.

This report outlines research undertaken by Alzheimer's Australia NSW (AlzNSW) in 2011 that sought to identify the service and support requirements of people with younger onset dementia and their families in New South Wales.

Younger onset dementia is defined as dementia which occurs before the age of 65 years and under 50 years for Indigenous people, and it is recognised that people with younger onset dementia have service requirements that differ from those of older people with dementia.

This research collected data from people with younger onset dementia, their family members and carers, and service providers, using roundtables, interviews and surveys. The findings describe the experiences of people with younger onset dementia and that of their families and carers, and are presented in three parts:

  • identifying younger onset dementia
  • experience of support services, and
  • services in the later stages of dementia.

The report concludes with a discussion of the implications of the findings for policy and presents strategies to address key areas of need.

This report presents the findings from the Alcohol action in rural communities project (AARC) conducted in 2005 - 2012. It evaluated the effectiveness of community action approaches to reducing risky alcohol consumption and related-harms in 10 communities across rural New South Wales, using 13 tailored strategies implemented over 5 years.

The project demonstrated that the benefits of implementing community action outweigh the associated costs of such action. Findings included:

  • 24% reduction in alcohol-related street offences
  • 8% reduction in assaults
  • 31% reduction of short-term high-risk drinkers.

The AARC project was a partnership between local communities, local government, government agencies, the Universities of New South Wales and Newcastle, and the National Drug and Alcohol Research Centre (NDARC), with funding provided by the Foundation for Alcohol Research and Education (FARE).

This resource package is part of a suite of health promotion resources for Aboriginal women and their families which aim to raise awareness about alcohol and other drugs and mental health issues during or following pregnancy. The materials promote the importance of staying healthy and avoiding alcohol and other drugs at this time. The Stay strong and healthy resource package includes:

  • three posters promoting Aboriginal women's health during pregnancy and after the baby is born
  • a postcard for Aboriginal women that promotes the availability of professional services and the role of friends and family in supporting pregnant women and women with young babies
  • a 16 page A5 size booklet with pictures that tells the story of a young Aboriginal woman who gets pregnant, and the healthy choices she and her partner make. The story discusses the risks of drug and alcohol consumption during pregnancy, including Fetal alcohol syndrome disorders (FASD), as well as the potential challenges of dealing with a mental illness.
  • Stay strong and healthy on Facebook; provides health information on pregnancy, looking after a new baby, drugs and alcohol and coping with feeling down. Local Aboriginal maternal and infant services are provided on the interactive map as well as healthy Koori recipes.

Evaluation of the NSW Indigenous Diabetic Foot Program for health workers whose primary role is with Aboriginal people in the lower mid north coast.

The Indigenous diabetic foot program (IDFP) provides educational programs for both Indigenous Health Workers (IHWs) and Indigenous patients who have diabetes. Components of the program include information on foot complications of diabetes, encouraging self-care and a screening tool for identifying those at high risk of foot complications (DART). This evaluation assessed the effectiveness of the education program for IHWs by measuring the knowledge gained by the IHWs and the integration of the screening tools into clinical practice.

Results showed that after the workshop IHW participants had increased their knowledge levels, and that this knowledge was sustained over at least a six month time period. Of particular note, the lowest score in the six month follow-up knowledge questionnaire was higher than the highest score in the pre-workshop knowledge questionnaire.

The study also found that while the participants found the IDFP to be beneficial, the uptake and implementation of the screening program was low (55%).

This toolkit is the deliverable output from the networking party. It provides a definition of 48 hour follow up (numerator/denominator), as well as outlining process steps required to be implemented to deliver a sustainable process. This toolkit must not be used as a definition of exactly how 48 hour follow up must operate in every facility. The reality is that every facility in every AHS is unique and will require local interpretation of the process steps identified.

The processes implemented across each AHS should be conducive to sustainable 48 hour follow up of Aboriginal patients with chronic disease, and not be dependant upon a specific group of individuals. It is intended that processes for 48 hour follow up of Aboriginal patients with chronic disease are integrated with mainstream services.

Further support in the development of 48 hour follow up processes will be delivered to AHS project teams via the standing Walgan Tilly project teleconferences and site visits.

The government healthy eating campaigns to combat the ‘gap' in health status between Indigenous and non-Indigenous Australians have tended to assume that the poor diets of Indigenous Australians and their subsequent poor health outcomes are because of their lack of knowledge about what foods are healthy. But lack of education is not always the problem. For many residents of remote communities it is the problems with supply and affordability of produce that limit the opportunities to consume fresh fruit and vegetables on a regular basis.

In 2006 the Rudd Government established a company called Outback Stores, to manage remote community stores on behalf of Indigenous communities. The purpose was to address poor management practices and reduce uneconomic cultural practices, which can result in the stores selling goods at high prices or providing inferior products and poor service.

In this report Sara Hudson, Policy Analyst in the Indigenous Affairs Research Program at The Centre for Independent Studies, discusses the impact of the Outback Stores on remote communities. She argues that the government involvement in Outback Stores has created an unequal playing field that stifles competition, and makes it more difficult for independent community stores to keep operating. She suggests that Outback Stores should not be allowed to operate in communities of over 500 residents and that government intervention into remote stores should be confined to monitoring and regulating stores practices.

The 10 out of 10 deadly health stories - nutrition and physical activity booklet presents 10 successful nutrition and physical activity programs from NSW Aboriginal Community Controlled Health Services (ACCHSs). Programs ranged from cooking classes (Cooking classes for diabetes program) and teaching of life skills (Healthy lifestyle and weight management program), to community based activities (Fruit and vegetable program and market garden and Building healthy communities project). Despite the diversity in health topics they all shared several features, namely all were developed and tailored to meet needs identified by the Aboriginal communities in which the ACCHSs are located, and all have taken a 'holistic' approach.

The booklet was developed, produced and distributed by the Aboriginal Health and Medical Research Council (AHMRC) of NSW and funding was provided by the Australian Department of Health and Ageing.

This monograph is a critique of the current National Indigenous health policy, coupled with recommendations on how a co-ordinated care model could deliver better health services to Indigenous communities. The author stresses the need for government to be more accountable and transparent in spending, and for Aboriginal community controlled health services (ACCHS) to demonstrate better governance and clinical effectiveness. The proposed co-ordinated model would draw on bottom-up consultation and top-down change processes to prioritise the health needs of individual patients.

These guidelines provide the direction for alcohol and other drugs (AOD) workers as they manage comorbid (occuring at the same time) AOD and mental health conditions faced by clients. They are not Aboriginal or Torres Strait Islander specific but do highlight the issues which need to be considered by AOD workers when working with Aboriginal and Torres Strait Islander people. The guidelines are based on the best available evidence, experience and knowledge of clinicians, researchers, consumers and carers. They aim to provide the AOD workforce with:

  • increased knowledge and awareness of mental health conditions
  • improved confidence and skills for working with clients with comorbid mental health conditions
  • guiding values for working with clients with comorbid mental health conditions
  • improved skills to enable them to identify mental health conditions
  • practical information on the management of comorbid mental health conditions
  • information on the treatment of comorbid mental health conditions

The Cultural respect and communication guide: a resource to assist sexual health service delivery to Aboriginal communities has been designed for health care professionals working in the area of Indigenous sexual health. It was developed in response to recommendations made in the NSW HIV/AIDS, sexually transmissible infections and hepatitis C strategies: implementation plan for Aboriginal people 2006-2009 and is the product of an extensive consultation process whereby project committee members, stakeholders, and staff from a range of service providers (e.g., local general practitioners, sexual health service staff) contributed to the resource content.

The guide seeks to enhance the knowledge and understanding of Indigenous culture among health care professionals so as to improve the delivery and accessibility of sexual health services to individuals and communities. Specifically the guide highlights the barriers to sexual health service delivery encountered by Indigenous peoples and offers suggestions for overcoming such barriers. Issues such as cultural respect, communication, shame, and access are addressed in this section. The guide also provides information on the process of community engagement and the different modes of service delivery.

These guidelines are for Aboriginal Family Health Workers in New South Wales (NSW).

The guidelines describe the following aspects of the Aboriginal Family Health Worker job in NSW:

  • planning
  • monitoring and evaluation
  • job description.

The guidelines specifically apply to Aboriginal Family Health Workers who deliver projects under the NSW Aboriginal family health strategy. The aim of the strategy is to reduce the incidence and prevalence of family violence within Aboriginal communities and to strengthen families and communities. Aboriginal family health projects were established in NSW between 1998-2001 and most are based in regional and rural areas.

This report documents the findings of a workshop held in Mildura that provided an opportunity to disseminate findings from the National Drug Law Enforcement Research Fund to an audience of NSW and Victorian police involved in implementing and managing the policing response to substance abuse in Indigenous communities.

Police presented environmental scans from their respective jurisdictions. The objectives of the workshop included: establishing the key issues for police in relation to substance abuse in Indigenous communities; identifying differences in policing illicit drug use in Indigenous communities compared with other communities; and identifying where police may be able to improve their response to these issues. Key findings included: research and environmental scans shared at the workshop raised awareness of the emerging issue of illicit drug use; differences between urban, regional and remote communities in policing responses to illicit substance use; differences in police responses to urban Indigenous drug use highlighted the need for further research; and networking between operational police and staff from other areas (such as Aboriginal Community Liaison Officers) is likely to provide significant benefits and needs to be encouraged.

This kit is a guide for general practitioners (GPs) for the effective delivery of health care to adolescents from culturally diverse backgrounds. The majority of the content is applicable to GPs working anywhere in Australia.

The skills and strategies outlined in the kit are organised into four sections:

  • understanding adolescents
  • skills for youth-friendly general practice
  • creating a youth-friendly general practice
  • youth health resources and contacts.

The kit includes templates for conducting:

  • adolescent health checks
  • youth health risk assessments (HEEADSSS)
  • youth-friendly practice reviews.

The kit takes a biopsychosocial approach to working with young people and their families. It encourages GPs to consider the developmental, cultural and environmental factors that impact on adolescent health.

The kit is endorsed by the Royal Australian College of General Practitioners (RACGP) and General Practice New South Wales (GP NSW).

In collaboration with the Aboriginal Health and Medical Research Council of NSW (AH&MRC), New South Wales Rural Doctors Network has developed and distributed an Administration Manual for Aboriginal Community Controlled Health Services (ACCHSs) in rural NSW.
The manual comprises two word documents. The first is instructions on how to use the manual and easily customise it to suit. The second is the manual itself which has been designed with links to web pages to ensure that as much information as possible remains current.
To help ACCHSs to develop their own job descriptions, Durri Aboriginal Corporation Medical Service has provided a list of job descriptions for general use.
