Managing two worlds together: stage 2 - patient journey mapping tools.

This paper describes the patient journey mapping tool and frameworks used in the Managing two worlds together (MTWT) project. Through interviews and focus groups with patients, their carer/family, and health care providers, the paper explores the barriers and enablers, gaps and strategies in relation to the journeys of country Aboriginal patients from rural and remote locations to city hospitals and their return. The primary aim of the MTWT project is to enhance existing knowledge of the strengths, and areas of improvement, in the care of Aboriginal patients from rural and remote areas of South Australia by exploring what happens when they come to Adelaide for hospital care. The project is comprised of two stages, with Stage 1 focusing on the problems, and Stage 2 focusing on solutions. The patient journey mapping tools are the first output of Stage 2.

Chronic condition management strategies in Aboriginal communities: final report 2011.

The main aim of the Chronic condition management strategies in Aboriginal communities project was to evaluate the effectiveness of tailoring mainstream chronic care management strategies to suit Indigenous clients and settings. This was achieved through assessing the health outcomes and impacts, and the sustainability of the strategies. The strategies included:

  • structured care planning focused on clients' problems and goals
  • training in chronic disease self management and management support for health staff and peer leaders
  • coordinated holistic team care.

Data collection involved the analysis of health service records of 36 clients involved in chronic care management over 1-10 years, and semi-structured interviews with 18 clients and 12 staff.

The research was conducted by a team from Flinders University and the Aboriginal Health Council of South Australia, in collaboration with Aboriginal clients and community members from Port Lincoln Aboriginal Health Service, Nunkuwarrin Yunti of South Australia Inc. and Riverland Commuity Health Service.

The authors conclude that people involved in structured chronic condition management strategies (eg care plans) improve their health and wellbeing over time. A range of barriers and enablers of chronic condition management strategies were identified and the authors highlight the importance of tailoring new initiatives to suit individual needs and local circumstances.

This document reports on a study to investigate whether the use of swimming pools by school-age Indigenous children in remote semi-arid communities in central Australia results in the reduction and possible prevention of conductive hearing loss related to otitis media (OM) in these children.

Between 2009 - 2011, multiple clinical assessments of ear health and hearing - including Ear, nose and throat (ENT) diagnoses - were conducted on 813 school age children, which allow comparison of the results for children living in the four Anangu communities with a swimming pool compared to those living in the six communities without a pool. Many children were assessed on multiple occasions with 46% of the children being assessed on at least three of the possible six occasions over the three year study period (2107 child assessments in total).

The study's results demonstrate that access to swimming pools does not result in improvement in any of three principal measured indices of hearing or ear health. Additionally, in relation to the policy of 'no school, no pool', there is no evidence that access to a swimming pool results in improved school attendance. Further, results show previously undescribed seasonal differences in the prevalence of eardrum perforations in these Indigenous populations. Other results show significant associations between dry eardrum perforation and hearing loss and age.

This research report was funded by the Hearing Loss Prevention Program of the Department of Health and Ageing and conducted by Flinders University and its collaborating partner, the Anangu Education Service of the South Australian (SA) Department of Education and Child Development.

This manual was designed to support Aboriginal Health Workers (AHWs) who perform health checks in South Australia for Aboriginal people with chronic diseases.

It was developed to increase the number of health checks being performed, help AHWs while performing adult health checks, and increase the number of community members who will benefit from these checks.

It includes information on:

  • pre-check and medical history
  • examination of patient
  • investigations
  • assessment of patient
  • equipment required.

The resource was developed by the Aboriginal Health Council of South Australia in collaboration with the South Australian Aboriginal Community Controlled Health Services and Flinders Far North Division of General Practice.

Draft South Australian tobacco control strategy 2011-2016.

Drug and Alcohol Services SA has produced a draft for public consultation of their new tobacco control strategy. South Australian tobacco control work is guided by several overarching documents that include obligations and strategies for further regulation of smoking in public areas and reducing the harms of second-hand smoke. The primary target within the South Australian tobacco control strategy 2011-2016 is to reduce the percentage of young cigarette smokers by 5.4% from 21.3% in 2009 to 15.9% in 2016. This extends South Australia's Strategic Plan Target 2.1 by two years and requires the same reduction of at least 1% per year starting from 2009.

The model of care describes the service level principles and processes that inform, underpin, and direct the provision of community mental health services to adults aged 18-64 years across the Adelaide metropolitan health regions. The document is not Aboriginal or Torres Strait Islander specific but does include information on Indigenous mental health.

The Adult community mental health services: model of care is intended for use in conjunction with the South Australian mental health and wellbeing policy 2010-2015 and may be of use as a reference for mental health workers.

This report looks at the Adelaide Aboriginal and Torres Strait Islander Health (AATSIH) study and aims to contribute to filling the gap in literature and knowledge about the health and everyday life experience of urban Indigenous peoples. The report aims to improve Indigenous urban health and empower Aboriginal individuals and their communities, by setting out the necessary changes to improve health care for Aboriginal and Torres Strait Islander peoples for the long term. The report highlights that although Aboriginal people feel positive about their neighbourhood and community life, racism is a common experience. The findings suggest that improved access to education, employment and housing is needed to enhance the everyday life experience of urban Indigenous people.

The thesis explored how the health and wellbeing of Aboriginal women within an urban Adelaide health care system could be improved. It utilised a collaborative qualitative research method. The research focused on how Aboriginal and non-Aboriginal health professionals could work within the parameters of the current health system to facilitate these improvements. The research highlighted the importance of finding practical ways of working together, respecting each other's knowledge and dealing with difficult issues together.

It promotes the need for prevention and education programs, proposes measures to be implemented in order to prevent child sexual abuse and to enhance child welfare and protection. It includes 46 policy implementation recommendations and provides an assessment of its related effects on the community.

Haemophilus influenza type B (Hib).

Hib is Haemophilus influenza type B it is a bacterial infection which can cause severe illness especially in young children. Before the introduction of Hib immunisation in 1993, the bacterial infection Haemophilus influenza type B was one of the most common causes of meningitis in young children (generally under the age of 2 years), and it was also the cause of epiglottitis which causes breathing problems. Australian Aboriginal and Torres Strait Islander children are at much greater risk of Hib diseases than non-Aboriginal children.

The Department of Families, Housing, Community Services and Indigenous Affairs (FaHCSIA) recently released the 'Review of first phase of the petrol sniffing strategy' prepared by Urbis. The Petrol Sniffing Strategy (PSS) involves an eight point plan that includes both supply reduction and demand reduction strategies which aims to:

  • reduce both incidence and impact of petrol sniffing in a defined area of Central Australia by addressing the social determinants of health and wellbeing
  • evaluate the effectiveness of a regional and comprehensive response to petrol sniffing to establish the utility of expanding the strategy to other regions.

Overall, stakeholders regard the eight point plan of the PSS as a realistic effort to address the petrol sniffing problem. The Opal fuel has succeeded in substantially reducing the level of petrol sniffing, though more needs to be done in relation to strengthening and supporting communities, and engaging young people. It is believed that improved consultation and sharing of information is needed, and that maintaining a focus on petrol sniffing is important until such time as it is sufficiently reduced, and then issues around sniffing can be seen in the broader context of drug and alcohol problems and their associated policy responses.
