Description

This resource has been designed to support Indigenous people with making advance care plans for future medical care and treatment, if they become seriously ill. The resource highlights the importance of yarning about and making choices on future treatment, so family or those appointed by the person, can yarn on their behalf and carry out their wishes. It advocates the need for effective planning and includes an Advance care plan, which is a written document to help people to accurately remember the person's choices. This makes it easier to communicate these wishes to doctors, nurses and Aboriginal Health Workers.
