Clinical practice guidelines and care pathways for people with dementia living in the community (2008)

Description

Dementia is an enduring chronic condition and within the decade, demand for access to quality dementia care is expected to increase exponentially. As such, these guidelines and pathways have been developed to ensure quality of care and improve health outcomes for people with dementia. The guidelines and pathways focus on non-pharmacological care and are intended for all health professionals and policy makers involved in the planning, organisation, and delivery of dementia care. The guidelines and pathways are presented in three phases: recognition, assessment, and diagnosis; post diagnosis, monitoring, management, and care phase; and the advanced phase. Where appropriate, information specific to Aboriginal and Torres Strait Islander people is included. The guidelines were developed in accordance with the principles suggested by the National Health and Medical Research Council, under the direction of a multidisciplinary panel and in consultation with key stakeholders and consumers.
