Description

The Type 2 diabetes risk assessment tool for Aboriginal Victorians is a tool for GPs, Aboriginal Health Workers and allied health workers to screen clients who may be at risk of diabetes. The risk assessment tool has ten questions relating to risk factors for diabetes. It also provides some information on how to interpret the results and what follow-up is recommended.
