Model of care for the high risk foot (2010)

Description

The aims of this Model of care are to prevent and/or delay complications of the high risk foot (high risk foot is a foot with progressive deformity, ulceration, infection and/or amputation and is most commonly associated with diabetes) at all stages, especially amputations and to deliver equitable and cost effective high risk foot services particularly to rural and remote Aboriginal communities.

The key recommendations of the Model are to:

The recommendations are intended to improve patients’ outcomes, provide more coordinated and efficient care and to reduce health care costs in the long term.
