Description

This workbook has been designed to be used by Indigenous people attending The journey of living with diabetes, a health education program developed by the South Metropolitan Area Health Service, WA, for Noongar people with type 2 diabetes.

The action plan workbook is divided into six sections which are likened to the Noongar six seasons:

Each section contains background information on the topic, and an action plan where the participant can enter their own goals and some ideas for activities that will help them achieve the goals.
