Description

This resource has been developed to improve the health outcomes of Aboriginal people in New South Wales (NSW). The resource aims to establish standards of practice for area health services for the prevention and management of chronic conditions in Aboriginal people. It is expected that areas will develop plans to meet the standards through local partnerships, in particular, local area Indigenous health partnerships, to ensure the input of Indigenous communities and Indigenous community controlled health services.

The standards augment the clinical service frameworks in heart failure, respiratory disease and cancer developed by the NSW chronic care program and takes a broader chronic conditions approach to include cardiovascular disease, diabetes, kidney disease and chronic respiratory disease.
