Mental health assessment form (2006)

Description

This two-page mental health assessment form has been developed for mental health practitioners with Aboriginal or Torres Strait Islander clients. Information is recorded on:

• client's history (family and cultural; psychiatric and forensic; and medical)
• the client's worries
• risk assessment
• mental state examination
• diagnosis
• treatment.

The mental health assessment form is part of a resource package developed by the Australian Integrated Mental Health Initiative.

When to use this resource

This assessment form is designed to be used by mental health practitioners when conducting mental health assessments on Aboriginal and Torres Strait Islander peoples.
