Fetal alcohol spectrum disorder booklet with questionnaire (nd)

Description

This leaflet provides information for the health professional on how to prepare for an assessment of Fetal alcohol spectrum disorder (FASD) in an adult. It includes a questionaire about FASD which should be completed by the adult being assessed, and samples of the type of information that should be collected to identify if the person is affected by FASD. The questionaire involves multi-choice questions.

When to use this resource

This resource is designed to be used by health workers to help them identify if adult clients have Fetal alcohol spectrum disorder (FASD). The health workers are not qualified to diagnose patients, but the information collected by the health workers will assist a doctor to make a diagnosis.
