Description

This toolkit was designed for use by Victorian organisations participating in the Aboriginal Health Promotion and Chronic Care (AHPACC) partnership initiative. It may also assist other organisations and partnerships implementing Aboriginal health programs.

The AHPACC initiative supports Aboriginal community-controlled health organisations (ACCHOs) and community health services to work in partnership to develop and deliver local services and programs that prevent and manage the high prevalence of chronic disease within Indigenous communities in Victoria.
