Transitions: release preparation program
Overview
Run by Queensland Corrective Services, the Transitions program is a release preparation program that provides offenders with an opportunity to address their practical re-settlement needs prior to release. The program provides accurate information, planning assistance, emotional support and linkages with appropriate community agencies. Transitions aims to support offenders' smooth re-entry to the community to reduce the likelihood of re-offending.
The key features of the Transition program include:
- five core modules identifying problems that could be faced post-release, consideration of strategies for addressing these problems, how to use community resources, developing realistic expectations, goal setting and problem solving
- eight elective modules that provide individualised needs assessment guides that address particular needs and reintegration issues
- the needs of Indigenous offenders and female offenders are specifically addressed in the manual, through facilitators' notes, suggested learning activities and identification of appropriate alternative resources
- workshop modules are facilitated by community-based agencies and government departments having specialist knowledge and expertise in relevant fields of service delivery and that can provide post-release support and assistance to offenders in the community
- a personal Transitions plan addressing self-identified release preparation and community re-settlement needs is developed.
