Prevent diabetes live life well program

 

Overview

The Prevent diabetes live life well program run by Tharawal Aboriginal Medical Service (AMS) was an adaptation of the Live life well program, originally developed by New South Wales (NSW) Health. The program provided support for clients aged 18 years and over, who were considered by their GP to be at risk of developing type 2 diabetes.

Participants received ongoing support from a specially trained Aboriginal lifestyle officer, teams of nurses, dieticians, exercise physiologists and other health professionals. They also learnt how to eat healthier, set personal physical activity goals, and overcome personal barriers to change.

Participants were encouraged to achieve five goals:
