Diabetes management and care program

 

Overview

The Diabetes management and care program is run in remote Indigenous communities in the Kimberley region of Western Australia by The Unity of First People of Australia, a not-for-profit Aboriginal-run organisation. The program is funded by Caritas Australia, The Catholic Agency for International Aid and Development.

The program aims to arrest the rising incidence of type 2 diabetes by working with the community to develop a sustainable community health plan which they own and manage themselves. The community health plan incorporates the prevention, management, treatment and care of diabetes and promotes lifestyle-changing behaviours for a healthier community.
