The Diabetes management and care program is run in remote Indigenous communities in the Kimberley region of Western Australia by The Unity of First People of Australia, a not-for-profit Aboriginal-run organisation. The program is funded by Caritas Australia, The Catholic Agency for International Aid and Development.
The program aims to arrest the rising incidence of Type 2 diabetes by working with the community to develop a sustainable community health plan which they own and manage themselves. The community health plan incorporates the prevention, management, treatment and care of diabetes and promotes lifestyle-changing behaviours for a healthier community.
