Chronic Illness Program



The Chronic Illness Program aims to increase awareness and understanding of the impact of lifestyle disease on people and families in local Aboriginal and Torres Strait Islander communities in the Northern Peninsula Area, Queensland.

The goal of the program is to achieve an overall reduction in the level of chronic illness in the community by informing the community of what can be done to avoid chronic illness and where to get assistance to change lifestyle habits.

The program is an initiative of the NPA Family and Community Services, who will also assist individuals to map out daily and weekly activities and motivate changes towards being more active.
