Chronic illness program

 

Overview

The Chronic illness program is an initiative of the Northern Peninsula Area (NPA) Family and Community Services. The aim of the program is to increase awareness and understanding of the impact of lifestyle disease on people and families in the local community.

The goal of the program is to achieve an overall reduction in the level of chronic illness in the community by informing the community of what can be done to avoid chronic illness and where to get assistance to change lifestyle habits.

The NPA Family and Community Services will also assist individuals to map out daily and weekly activities and motivate changes towards being more active.
