Chronic disease care co-ordination and supplementary services

 

Overview

The Chronic disease care coordination and supplementary services program is aimed at supporting Indigenous people with chronic diseases across the Southern Downs area of Queensland. The program aims to help manage chronic diseases with screening for early detection and treatment.

The service offers Indigenous people who have cancer, diabetes, chronic heart, lung or kidney diseases, with care plans which includes five free allied health appointments such as physiotherapy or podiatrist appointments.
