Care coordination for Aboriginal and Torres Strait Islanders



The Care coordination for Aboriginal and Torres Strait Islanders program aimed to identify and contribute to improved health outcomes for Indigenous people, and reduce the number of emergency and acute care admissions.

The program employed care coordinators to work with general practitioners in developing chronic disease management plans for their Indigenous patients, assist the patient in accessing allied health, specialists services, and support them with chronic disease self management.

The program also employed Closing the Gap Aboriginal Outreach Workers who assisted patients by providing transportation, advocacy and support to attend health appointments, and assistance with pharmacy.

This program was an initiative of the Northern Adelaide Medicare Local. Northern Adelaide Medicare Local ceased operation in 2015, and was replaced by Adelaide PHN, an advocacy service for local health organisation in Southern Adelaide, Fleurieu, Kangaroo Island, Central Adelaide Hills and Northern Adelaide.
