The Prison health re-entry program targets Aboriginal and Torres Strait Islander prisoners within the Goldfields Region of Western Australia. Workers aim to provide prisoners and their partners and families (where appropriate) with a holistic 'through-care' service encompassing primary healthcare, social emotional wellbeing and drug and alcohol services to assist in breaking the cycle of offending.
This program was funded under the Closing the Gap priority area of 'Making Indigenous health everyone's business' by supporting prisoners in a healthier transition from prison back in to the family and community.
