The Prison Health Re-Entry Program targeted Aboriginal and Torres Strait Islander prisoners within the Goldfields Region of Western Australia. Program workers aimed to provide prisoners, their partners and families (where appropriate) with a holistic 'through-care' service encompassing primary healthcare, social emotional wellbeing and alcohol and other drugs services to assist in breaking the cycle of offending.
This program was funded under the Closing the Gap priority area of Making Indigenous health everyone's business by supporting prisoners in a healthier transition from prison back in to the family and community.
