The Chronic Disease Self Management Support Program

 

Overview

The Chronic Disease Self Management Support Program is a short course offered in Queensland (Qld), New South Wales (NSW), Victoria (Vic), Australian Capital Territory (ACT) and Western Australia (WA) designed to develop the knowledge and skills required to support an individual or client with a chronic disease through:

The program is introduction for those new to chronic disease support or those wishing to enhance or refresh their knowledge and skills. Locations and dates are:

On successful completion of the program, students should have the ability to support qualified Health Practitioners, and allied health workers, to help people managing chronic disease.

The program is driven by a desire to create better health for clients, reduced hospital admissions, better patient compliance and the provision of care by those best qualified to provide advice.
