Abstract

This resource describes a care pathway for the establishment of an integrated patient centred multi-disciplinary renal palliative care service. The report outlines the education and training requirements, identified educations tools, curriculum content and delivery methods to prove clinicians with the necessary skills and knowledge to facilitate the necessary discussions and administer the best practice on-going symptom management through end of life care.
