2005
Derbarl Yerrigan Health Service – Chronic Disease CIP Project
Run in conjunction with a Commonwealth-funded health initiative
this program was developed in several stages. Initially the
identification of chronic disease profiles within the Indigenous
population was undertaken, followed by a second stage which
incorporated development of policies, protocols, and a manual
to support chronic disease management.
