The Northern Territory chronic conditions prevention and management strategy implementation plan 2010-2020 focuses on the development of a framework for a 'system wide' approach to tackling chronic conditions in the Northern Territory (NT). The strategy has a ten year time frame and a particular focus on cardiovascular disease, rheumatic heart disease, type 2 diabetes, respiratory disease, kidney disease, cancer and mental illness. The plan, which will be reviewed and updated every three years, provides an outline of strategies and actions required in the NT. It lists key elements and key action areas which will be measured in order to determine if the strategy's aim and subsequent goals, to improve the health of all Territorians, are being met.
